FREE MEDICARE PART D ASSISTANCE
Enrollment and review assistance for Medicare Part D Plan for prescription drugs plans will be offered by a Central Illinois Agency on Aging Volunteer, by appointment at the Morton Public Library.
Mel Clauser, SHIP Counselor and Central Illinois Agency on Aging Volunteer, will be available to assist you by appointment at the Morton Public Library. Appointment times will be on Wednesdays, October 18-December 6 between 9:30 am and 3 pm and will last one hour. There is no charge for this service. Call the Morton Library at 309-263-2200 to schedule an appointment. Below is a list of information to bring with you to your appointment:
Medicare card
A list of all your prescription medications (including medication name, dosage, and frequency taken)
Your MyMedicare Account Username and Password, if you have previously set up a MyMedicare account where you can view your personal Medicare information online.
If you do not have a MyMedicare Account set up, we will need to do that during your appointment.
